Registration Form

Downloaded forms should be printed and hand-filled. All Completed Forms should be signed, and submitted to the Principal, Monef High School, or Monef Kiddies School, along with the nonrefundable fee and copies of the required documents:

  • Birth Certificate (Photocopy)
  • Immunization Card (Photocopy)
  • One (1) passport photograph
